A late strike by Ivan Calero saw Charlton go out of the Capital One Cup at Derby on Tuesday night.
The second-round tie was heading for extra-time until Calero finished a pass from Chris Martin in the 87th minute.
Addicks beat Derby in the Championship earlier this month but Bob Peeters' weakened team barely threatened in the cup.
Steve McClaren's side went close to opening the scoring when Jeff Hendrick volleyed wide after Richard Keogh met a corner.
Leon Best also forced a decent save from Charlton keeper Nick Pope, while the visitors' best effort saw George Tucudean's strike blocked by Keogh.
